Cancellation policy

Please note that Rachel requires 24-hour notice if you need to change or cancel your appointment. Call or email to cancel or reschedule at least 24 hours prior to your appointment time to avoid charges. Rachel realizes life happens and give every active client one “freebie” late cancellation or no show. With the exception of true emergencies, active clients will incur the full fee for every late cancellation or no show thereafter. Rachel appreciates your cooperation with this policy.

Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Insurance coverage

Rachel Dubrow, LCSW accepts self pay clients as well as those wanting to use Lyra health benefits.

​All fees are due at the beginning of each session via cash, check, or Zelle.

USING OUT OF NETWORK BENEFITS:

 

Rachel sees several clients in the practice who want to use out of network benefits. They choose to use their out-of-network benefits because the reimbursement to clients is comparable to what they might pay as a co-pay. To use your out-of-network benefits, you would pay our full fee and would receive a superbill to submit to insurance directly. The superbill has everything that is required by the insurance companies to adequately process your claim. 

If you choose to utilize your insurance for out-of-network coverage, Rachel encourages you to contact your insurance company and ask the following questions:

  • Do I have mental health coverage?

  • What is the out-of-network coverage for CPT codes 90791 (Initial Evaluation) and 90834 (Psychotherapy)?

  • How many sessions will be covered per calendar year?

  • What is my out-of-network deductible? Has it been met?
